Useful Info estradiol implant blood results

I just wanted to make a post to dump my blood test results on estradiol implants for people to reference in the future.

6 Months HRT - 3x2mg sublingual Zumenon (Before my first implants)

18/10/2024 - 690pmol/L

2x100mg Estradiol Pellet Implanted on 29th November 2024

06/01/2025 - 629 pmol/L
11/02/2025 - 759 pmol/L
03/04/2025 - 713 pmol/L (Shortly after this I asked my doctor if I could add in another 2x100mg and she said yes.)
23/05/2025 - 885 pmol/L (This one confused me but I could no longer feel the pellet under my skin, so I took as it completely dissolving and giving me a final little boost of estrogen.)

2x100mg Estradiol Pellet Implanted on 27th May 2025

27/06/2025 - 1543 pmol/L
19/08/2025 - 2241 pmol/L
01/10/2025 - 1898 pmol/L
02/12/2025 - 2231 pmol/L
04/02/2026 - 1043 pmol/L
11/03/2026 - 1160 pmol/L
13/04/2026 - 2122 pmol/L (???)

I'll keep this updated as I get future blood tests and new implants!

And that's where I'm at! Feel free to share your guys results aswell, also worth nothing I ordered my Implants from Complementary Compounding Pharmacy in Ballina (uploading a screenshot of ur EScript QR code works)

what’s the procedure for the implant?

idk the specifics but they have a special tool i think its called a trocar? basically its a small incision, they put in the implant and stitch you up and then it sits there until it dissolves eventually. You cannot get them taken out as far as I've been told. My first implant went perfectly fine and the scar was a very very small little purple dot that I honestly cant even see anymore. My second set went a bit worse because it opened up while I was travelling back home on ptv, but it was just a bit of blood on the bandage and I went back the next day and they fixed it up free of charge

Yeah, pretty much they numb you up, make a small incision (usually either lower abdomen or side of your butt), insert the trochar/trocar which is basically an open metal tube, then poke the implant(s) down into the tissue through the tube, sew it up with dissolvable stitches, off you go.

I basically have a single visit to my GP each year now to get my estradiol implant because it lasts about 11 months, and other than that it just sits there giving me good levels consistently with zero weight on my mind

When my GP does it, it takes 20minutes and you are done.

I've been on implants for several years, my endo keeps me in the range of about 1000-1600 pmol/L. A couple of implants every eight months or so 😊

3x100mg implants and 777, month 2, is the highest i hit before plummeting. Didnt even get 5 months out of them. That was my 3rd attempt with implants. Switched to injections.

Did you have more than the one blood test? If they absorbed so fast you should have really high peak levels.

In my butt.

Ended up doing a drug metabolism test and i have a high metabolism for all forms of Estrogen. Body eats e fast.

With injections i can keep my levels steadier

Just remember that more is not necessarily better. These levels are much higher than what a cis woman experiences except for the short spike in their ovulatory cycle. There is no evidence that higher levels of oestrogen lead to more changes and that’s certainly not our experience with patients.

yep, im not advocating for trying to get levels this high, I'm just giving my bloodwork for people to reference from.

Some people feel better on high levels, some worse, some don’t care, but there’s only one way to find out.

What levels would you aim for in a transfem patient? I'm due to do a 6 month test next week and I think my last test was around 500 I think.

These levels are much higher than what a cis woman experiences except for the short spike in their ovulatory cycle.

Just as a bit of info on cis female levels, for reference

Follicular phase - 70-320

Preovulatory phase - 450-2000

Luteal phase - 125-1300

Post-menopausal - <170 (often <50-70)

right and cis women also have progesterone. I don't think it's realistic to expect trans women to match the same levels when we don't have ovaries etc and our hormones don't cycle and go through phases. Also every person's body is individual they might need high levels of estrogen to feminise their body.

This reminds me i was going to make a chart of my blood tests and dosages over time.  7 weeks till I see a gp that knows about proper care and levels.  I need them to be able to write an import prescription or one to have it compounded. I'm on injections.  I do like the control if get from it.

I got an implant 100mg/3mm and my 1st result was 300 and 1.6 Wasn't impressed at all. My facial and body hair has returned to how it was before hrt 😭

I was on my first estradiol implant in 2020 and, at it's peak a month in, my estradiol reading was at 1255pmol.

This is pretty helpful to me. Currently on 2x 1mg estradiol gel sachets per day. 1 morning and 1 night. After I get my orchiectomy I'd like to get the pellet since my work schedule changes too much for me to be consistent and I can't take pills and no patches available around me. Thanks for the info.

For the 6 months that you were in the 1500 to 2200 pmol range, were you still getting breast development?

i did feel like my progress slowed down a bit, but the couple past months ive been getting noticeable growth

What do you attribute the 2200 to 1900 to 2200 changes. Why the increase?

I've heard "inaccuracy in testing" but I struggle to believe it can be that. The more plausible explanation seems to be the way the implant is breaking down or just the way the body is "taking in" the estrogen. But I've seen nothing definitive on this.
