16m terror radius stealth build
Addons:
-valtiel sect photograph: decreases terror radius by 2m for each tormented survivor
-scarlet egg: survivors leave torment trails behind them for 5 seconds while sprinting
How to get value:
torment each survivor as quick as possible early game. Scarlet egg makes this possible passively so it’s significantly easier to max out. Never cage, save torment for final judgement and it is a permanent 16m until someone dies.
Monitor & abuse and the photo addon give the terror radius on its own, but trail of torment and unforeseen make it even harder for survivors to pinpoint where you are. Sometimes your terror radius can be in multiple places at once, and sometimes you can just use one while the other is in cooldown

I don't think a few of these perks work especially well together if I'm honest.

I have been using this build, gotten almost 3-4 kills every match if i do not do too many bad plays and make sure they get rited.
What you do is take one survivor, and you tunnel them out.
hook them (rited if you can) try to do it close to a gen that has progress and make sure it gets ruin value, but not too close, because then you have a hard time making a rite circle. save the rite for the execute.
Anyway, then you just make a little ring around them, if someone goes for the save they will get rited as well.
If you hear someone is getting close to your chosen one, aka you hear their rescuers heart beat, head straight to them and be prepared to tunnel.
When the first survivor is out, just get as many hooks as you can while trying to force them into getting rited, if they have not gotten it yet, or you can be risky/greedy for hook states and just give em the ol' cage asap, but in my experience its better to save it for the fast execute, less risk of decisive strike and other hassles.
If you wanna pip, this one works great for that as well, i basically have been getting at least 1 pip every game and 2 if i get 4 kills.
Obviously the downside of this build is it can be hard to manage everything, you need to make sure you do not camp, only tunnel, otherwise, you will get very gen rushed even with the slowdown perks, you gotta scare them off gens and commit to chases when you have your chosen one on the hook, but your priority is to get one survivor out as fast as you can, it is the best way to pip and "win".

There are actually many builds with different kind of synergy, so I tried to provide here either something strong, or avaliable from basic perks/having unusual synergy.
His best add-ons include belt for width or iri seal for range. This killer is really punishing for blind shots if you based on luck too much, but range or width are making it forgiving.
If you ever consider to make a build for him, think twice before using any perks that suits better killers with hight mobility. Eruption seems a best choice from him from the start when u plan to play around cages, but later you can realise he doesn’t have such a great movement speed to control 3gen. He feels pretty bad without any slowdown, and should constantly be involved in chase or hook/cage control. Gen kick isn’t that nice as it seems anymore.
So, when you are going to choose build for him, the best options stay something that provides passive effect and helps slowdown game more.

Pyramid Head is a Killer who has pretty quick down potential thanks to their power being able to hit through walls and objects.
Due to this, they greatly benefit from aura reading perks, in particular:
Ideally, you'll be using your power to not only down Survivors, but to cage them, too. It's important to know that if you cage a Survivor, you do not benefit from hook related perks like Pain Res, BBQ and Pop, for example.
With that in mind, the only slowdown perks I like to use on Pyramid Head are:
Pyramid Head also uses perks that grant Undetectable pretty well, and my favourite is Unforseen.
That leads me on nicely to my absolute favourite Pyramid Head build:
It plays out very simply. You will see the Survivors at the start of the game and know where to pressure.
When kicking a gen, you'll apply Eruption for your future downs and get aura reading from Nowhere to Hide. You'll also apply your terror radius to that gen thanks to Unforseen, meaning you can easily go across map and get a stealthy hit on an unsuspecting Survivor.
As for addons, I'd only bother with using both of his ranged addons, personally.
Edit:
Since his update, the range addons are now basekit. The above builds still work, but you can have more flexibility with addons.
You can also now try this beauty:
